Thank you very much for taking the time to fill out this questionnaire which we send to all referral partners to ask how your beneficiaries are getting on.

Whether we helped with devices and data or offered support, it’s really valuable to us to understand what’s working well and where we can improve.

Our brief questionnaire should only take 10 mins to complete, and covers:

  • a summary of the devices/support you received from us
  • any ways we helped your clients with education or job-seeking
  • any benefits they felt with inclusion or wellbeing
  • anything that was unexpectedly good or didn’t work out right
  • final feedback and chance to contribute to case studies
